Everyone at some point experiences problems with memory, attention, or concentration, but these problems may occur more frequently for individuals with neurologic diseases like MS. The following questions describe several situations in which a person may encounter problems with memory, attention or concentration. If you are marking your own answers, please circle the appropriate response (0, 1, 2,...) based on your cognitive function during the past 4 weeks.
